An observation sheet states which facts count and which facts stay outside.
Each observation has a source and a date. Exclusions sit on the same page as inclusions, so a reader does not treat a left-out fact as a used fact. This page does not grade a market price, and it does not call an observation a clinical test. If a source cannot be checked, that line stays off the sheet.
